COVID-19 vaccinations for children aged 12 to 16 years of age 

The Immunisations Team will soon be offering 1st and 2nd dose COVID-19 vaccinations in school. We would like to ask if you wish to give your consent for your child to receive this vaccine.

The main purpose of the COVID-19 school age vaccination programme is to provide protection to the children who receive the vaccine as this may help to reduce transmission of COVID-19 in the wider community.
We will be offering the Pfizer BioNTech (Comirnaty) mRNA vaccine. Please use this QR code to access the secure online consent form for your child. Please add the child’s NHS number if you are able. 
Please indicate your decision regarding consent by completing the online form.

We would ask that you let us know your decision indicating either YES or NO, it is important we have as many forms returned as possible. Consent Form
By consenting you and your child will be helping to play your part in reducing the risk of COVID-19 spreading. For your child to receive the 2nd dose of the vaccine it must be a minimum of 12 weeks since they received the 1st dose, additionally, if your child has had a positive PCR test or positive LFT test for COVID 19, unfortunately they must now wait a minimum of 12 weeks from the date of the test until they are able to receive the vaccination.
When we visit school, it is extremely important that you encourage your child to both eat and drink before attending the session, it is also important that they wear a short-sleeved shirt, or a t-shirt under their shirt to ensure their privacy and dignity at the sessions.
